How to Request a Florida Birth Certificate

Get the current form from floridahealth.gov. Only use official state forms — third-party forms cause delays.
- Complete the form with full birth details
Full name at birth, date of birth, county of birth, and both parents' full names (including mother's maiden name) are required.
- Prepare a copy of your government-issued photo ID
