Which state holds the record?

Select the state where the event occurred — not where you currently live. Birth, death, and marriage records are held by the state where the event took place.

Your Checklist

What to Prepare — Check Off Each Item

Disclaimer: This checklist is a general guide based on typical requirements. Always verify current fees and exact requirements directly with the government office before submitting. Requirements can vary by county within a state.

    How to Use This Checklist

    1. Gather every item before submitting

Missing one item gets your request rejected or placed on hold. Assemble everything first, then submit once.

    2. Download the official state form only

Go directly to the state government website for the form. Third-party forms are not accepted and will cause delays.

    3. Order extra copies — now

The checklist shows the per-copy fee. Order more than you think you need. Each institution you deal with needs its own certified original. Re-ordering later costs the same but means a second wait.

    4. Send by certified mail for anything important

Certified mail with return receipt gives you proof of delivery if the application gets lost. For urgent requests, go in person to the county office for same-day service.
